Track the work being done with clients towards predetermined outcomes

You can easily record when your work is related directly to specific outcomes with Outcome tracking. Outcomes can be assigned to client and whānau session records to track work towards predefined outcomes. You can also track the reasons for adding and removing the outcomes that you are working toward with clients and whānau.

Please note: Outcome tracking is only available for specific organisations while we refine this feature. If you would like to discuss this, please get in touch.

Creating and editing outcomes

Outcomes are set in Administration according to your organisation’s needs.

To create or edit outcomes:

  1. Navigate to Administration, using the Switcher

  2. Select Organisation, then Outcome tracking from the menu

  3. Select the Outcome you wish to edit or click Create outcome to track

  4. Slide the toggle to indicate whether the outcome is enabled

  5. Add a name and description for your outcome then click Save outcome

You can edit the name or description of your outcome at any time. You can also enable or disable this outcome for your organisation as required.

Outcome reasons

When adding or removing Outcomes, a user can select a reason for this change. These reasons feed in to reporting and can be customised within Administration to reflect your organisation.

To edit existing reasons or create a new reason:

1. Navigate to Administration, using the Switcher

2. Select Organisation, then Outcome reasons from the menu

3. Select the Outcome you wish to edit or click Create outcome reason

4. Add a name and type for your Outcome reason

5. Slide the toggle to indicate whether the reason is enabled, and click save

Assigning outcomes to a client and whānau

Once you have set up your organisation’s Outcomes these need to be assigned to a client or a whānau in order to track outcomes against records within Noted.

Assigning Outcomes to clients

  1. Edit the client you would like to assign outcomes to, or create a new client

  2. Select Outcome tracking from the left

  3. Assign the appropriate outcome or outcomes to the client by clicking the + symbol next to the Outcome

  4. Select the outcome reason, and amend the date if required

Assigning Outcomes to Whānau

  1. Edit the whānau you would like to assign outcomes to, or create a new whānau

  2. Select Outcome tracking from the left

  3. Assign the appropriate outcome or outcomes to the client by clicking the + symbol next to the Outcome

  4. Select the outcome reason, and amend the date if required

Tip: to remove an outcome from a client or whānau, simply click the ‘x’ next to the relevant Outcome when editing the client/whānau.Select the reason for removing your outcome and amend the date if required.

Adding outcomes to client records

When creating or editing records you can easily add outcomes from your predetermined list. If you cannot see the outcomes you are expecting to see listed, check that all the appropriate outcomes have been added to this client by editing their details and navigating to Outcome tracking.

To assign an outcome to a record

  1. Create or edit an existing record

  2. Select the appropriate outcome for the list under outcome tracking

  3. Enter the rest of the record’s information and when finished click Save and review
