Noted uses a collection of data domains and standard forms to classify and organise the data points you collect. This helps you get the most out of your data in Data Discovery. The below table gives you an overview of the different standard forms you can have.
Standard form | Definition/Usage |
Activity session | An activity session is a limited time interval during which one or more workers do work with, or on behalf of, a Client, Whanaunga āwhina or Group. |
Observations and evaluation form | Records observations and evaluations. It's used together with an Activity session form. |
Comprehensive assessment | Works like the Observations and evaluation form but lets you collect data over time. Use it when a full assessment is too long to finish in one sitting. |
Assessment tool | Records screenings. One tool can include several sub-screenings. |
Contingency plan | Records contingency plans, safety plans and similar plans. |
Client descriptor(s) | Records extra Client profile information that the standard Client profile doesn't cover. You can make this information visible only to certain teams. |
Request | Records requests a team sends to other teams in the organisation or to outside organisations. |
Team pathway | Tracks a Client/Whanaunga āwhina journey with a team. It moves through set statuses that mark key milestones, like the first contact or the discharge. It starts as soon as the team begins working with them, even if that's just planning the first meeting. It ends when the team's work is finished, for example at discharge. |
Wellbeing pathway | A plan of actions to assess or improve a given wellbeing aspect(s), showing which actions are done. It has two phases: |
Financial pathway | Tracks how approved funding is paid out, including follow-ups with the requester or the Client/Whanaunga āwhina. It moves through set statuses that mark milestones as the money is paid out. It starts when a funding request is approved and ends when all funds are paid and no more follow-ups are needed. |
